A release of compressed hydrogen and subsequent fire occurred during the transportation of truck tractor chassis with a mounted trailer module. The tube trailer module contained 25 fully wrapped carbon filler reinforced aluminum-lined cylinders, 24 of which were fully loaded with 240 kilograms of compressed hydrogen. The module caught fire in the neighbourhood of a hydrogen refuelling station. The pressure relief devices actuated on 12 of the cylinders and released about 120 kilograms of hydrogen that was likely consumed in the fire.

A major leak (estimated between 1.5 and 3 kg) of hydrogen from high-pressure storage, resulted in a detonation, from hydrogen a refuelling station.Causes were two untightened bolts securing the gasket between the high-pressure tank and the pipeline.

A petroleum refinery experienced a catastrophic rupture of a heat exchanger in the catalytic reformer / naphtha hydrotreater unit (the NHT unit). The heat exchanger, catastrophically ruptured because of High Temperature Hydrogen Attack.Highly flammable hydrogen and naphtha at more than 500 degrees Fahrenheit (260C) were released from the ruptured heat exchanger and ignited, causing an explosion and an intense fire that burned for more than three hours.

A hydrogen and fuel cells bus experienced a hydrogen detection alarm in a city centre. This triggered the evacuation of the immediate surroundings and the towing of the vehicle to the maintenance garage.

This near miss occurred at a CHP plant operation with a SOFC. During the heating phase, the SOFC stopped due to the opening of the mainpower switch.This was due to a contact problem inside the SOFC module, but the reason was not clear at first.

A vehicle overlooked into a road, overlooking the presence of another vehicles. The latter tried to avoid the accident by crossing to the other lane, occupied by a FCEV. The car was hit in the front.

The FCEV driver did hit a pole with the left front at a relative low speed.

A truck conventional truck moved to another lane and did not see the FCEV. The collision caused a total loss to the latter.

A conventional truck drove into a FCEV. The left rear side was damaged. There were no injuries and no hydrogen released. The car body needed to be repaired and the receptacle and the hydrogen tube need to be replaced.

The accident occurred in the Reactor 6 used to carry out the hydrogenation of a pharmaceutical intermediate.The operation of the plant was organized in two shifts. Since the hydrogenation at Reactor 6 would have taken at least 30 hours, it was not possible to complete the reaction during a single shift. The reaction was commenced at 8:00 a.m.
