The accident originated in the isomerisation unit and propagated to the benzene saturation unit. The accident source was a mechanical rupture of one of the two compressors used to transfer hydrogen mixtures to the isomerisation unit . The rupture caused the release of a gas mixture containing hydrogen, which ignited by mixing with air, forming a jet fire. The jet fire headed was directed towards the adjacent benzene saturation unit, hitting a pressure vessel located 14 m from the release point (the jet flame could have been 30 m long in absence of obstacle).

This near miss occurred at a CHP plant operation with a SOFC. During the heating phase, the SOFC stopped due to the opening of the mainpower switch.This was due to a contact problem inside the SOFC module, but the reason was not clear at first.

An explosion occurred around noon while conducting hot work (with a disc saw) on a 320-m fixed-roof tank at a pharmaceutical plant employing 240 personnel. The tank belongs to a waste treatment unit as part of an antibiotic production process. The tank cover was blasted 20 m away and the subcontractor performing cutting works was killed. Three other subcontractors and a plant employee were severely burnt and injured by the shock wave. Notified by neighbours and not by the operator, fire fighters arrived on the scene 15 min after the explosion and promptly extinguished the fire.

The event occurred at a de-sulfurisation reactor unit. Hydrogen leaked into the reactor causing an explosion. The Fire spread rapidly.

A fire broke out on a heat exchanger in a hydrocarbons plant. The cause of the fire was leakage of hydrogen.

The incident aoccurred ina biogas-fed Solid Oxide Fuel Cell Stationary system, working in CHP mode.A sudden stop of the biogas feeding line caused the safety shutdown of the plant (SOFC module).The reason was ice formation inside the chiller (located after the biogas compressor) and inside the condensing discharge piping. This ice was creating a pressure drop across the compressor, which went into alarm mode. By the following maintenance, more heating power has been installed on the pipelines and the ventilation speed was reduced during winter time.
