The event occurred in a bus garage when carrying out the purging process of the hydrogen cylinders. This process was executed via an electronic control system, but it was found that one cylinder solenoid valve was mechanically stuck. In these cases, the valve manufacturer has foreseen an alternative manual process using an override tool. The process was executed according to manufacturers instruction, but the valve did not start to evacuate hydrogen when expected. The fuel cell engineer tried to rectify the problem and started removing the override tool, but forgot to unwind the stem.

A hydrogen and fuel cells bus experienced a hydrogen detection alarm in a city centre. This triggered the evacuation of the immediate surroundings and the towing of the vehicle to the maintenance garage.

This near miss occurred at a CHP plant operation with a SOFC. During the heating phase, the SOFC stopped due to the opening of the mainpower switch.This was due to a contact problem inside the SOFC module, but the reason was not clear at first.

A vehicle overlooked into a road, overlooking the presence of another vehicles. The latter tried to avoid the accident by crossing to the other lane, occupied by a FCEV. The car was hit in the front.

The FCEV driver did hit a pole with the left front at a relative low speed.

A truck conventional truck moved to another lane and did not see the FCEV. The collision caused a total loss to the latter.

A conventional truck drove into a FCEV. The left rear side was damaged. There were no injuries and no hydrogen released. The car body needed to be repaired and the receptacle and the hydrogen tube need to be replaced.

At a linear accelerator site, a hydrogen retort exploded during ignition of burn-off gas. One employee experienced hearing loss in one ear and a shoulder injury from being blown into a cabinet. A second employee was checked by medicals and authorized to return to work.
