A maintenance plan and the results of inspection and maintenance should be documented and stewarded. All maintenance should be systematically recorded to show what was done, by whom, when it was completed, etc.

Root causes of any problems should be identified, documented, and action taken to prevent similar problems from occurring in the future.

Maintenance and testing documentation should reflect the current system configuration. All modifications should be documented and reviewed.

Management should emphasize to all personnel that current procedures must be followed. If a work order is not written to reflect the current system configuration or current procedures, maintenance staff should stop work (i.e., see Safety Culture: Formalized Procedures), revise the work order, and have it properly reviewed and approved prior to continuing the work.

If problems are identified during maintenance that could have resulted in a dangerous situation for personnel or equipment, please consider reporting them as "near misses" to H2LL.
